Qualifications

• Minimum 1 year of home health experience.
• Excellent computer skills.
• Excellent interpersonal and organizational skills.
• Phone etiquette skills.
• Typing speed 40-50 WPM.
• Knowledge of Medicare regulatory guidelines.
• Graduate of an accredited school of practical/vocational nursing.
• Current LPN license.
• Current state's driver’s license.
• Reliable transportation and auto liability insurance is required.

Responsibilities

• Schedule and document patient schedules in Home Care Home Base software.
• Coordinate care with management and interdisciplinary team.
• Provide effective communication to patients, staff, and other health care professionals and referral sources.
