Job summary

Cedarhurst Living seeks an LPN/RN to deliver and coordinate high-quality resident care under physician orders and plans. You will guide care staff, promote safety, and ensure clear communication with families, physicians, and leadership.

Responsibilities include medication administration, assessments, documentation, and direct resident support, with a focus on compassionate, resident-centered care in a collaborative team environment.

Qualifications

• Graduation from an accredited nursing school and a current state license for LPN or RN is required at all times.
• 1+ years of nursing experience preferred but new graduates are welcome.
• CPR or BLS certification preferred.
• Demonstrated proficiency in common digital tools and applications, including Microsoft Office programs (Outlook, Excel, Word), web-based platforms, electronic health record systems, and any HR-related systems as applicable.

Responsibilities

• Provide nursing care in accordance with nurse practice act and center regulations.
• Work with the Director of Nursing to oversee care staff and resident needs.
• Review and revise assessments and care plans at admission and after changes in condition.
• Administer and document medications as prescribed and monitor for safety.
• Monitor residents' nutrition and hydration and update records accordingly.
• Assist residents with activities of daily living and maintain a safe environment.
• Coordinate scheduling of medical, dental, and laboratory appointments.
