Job summary

Clinton Place is seeking a compassionate LPN/RN to join our care team on a 7a-7p shift. You’ll deliver direct nursing care, administer medications, monitor residents, and collaborate with the healthcare team to plan and evaluate care plans.

Qualifications

• Current LPN or RN license in State.
• At least one year of experience as an LPN or RN in a Long Term Care setting preferred.
• Knowledge of Matrixcare preferred.

Responsibilities

• Provide direct and individualized nursing care to residents.
• Administer medications and document as prescribed.
• Record vital signs and monitor patient status.
• Manage bedside care and assist with procedures and treatments.
• Coordinate with healthcare team to assess and plan care.
