Job summary

Parham Health & Rehabilitation Center in Richmond, VA is seeking a qualified LPN for 7a-7p and 7p-7a shifts. You will work as part of a nursing team to provide compassionate resident care and collaborate with healthcare professionals to ensure high-quality outcomes.

You will administer medications, monitor vitals, and document nursing observations while supporting residents’ independence and dignity throughout their stay.

Qualifications

• Active, unencumbered LPN license in Virginia.
• Excellent communication and interpersonal skills.
• Highly organized and detail oriented.
• Ability to work independently or as part of a team.

Responsibilities

• Supervise, coordinate, and direct the work of CNAs.
• Provide individualized nursing care per resident plans.
• Assist in admission, transfer, and discharge of patients.
• Administer medications and treatments as ordered by physician.
• Explain procedures to gain patient cooperation and reduce anxiety.
• Observe vital signs and monitor progress.
• Maintain and record nursing observations and actions.
• Record care plan details for continuity of care.
• Support emotional well-being and prepare for continuing care after discharge.
• Perform other duties to provide exceptional resident care.
