Qualifications

• Current state LPN license and CPR certification.
• Graduate of an approved LPN program per state requirements.
• 1–2 years of nursing experience preferred.
• Experience with elderly care desirable.

Responsibilities

• Assess, communicate, and document residents' status.
• Observe and report significant changes in resident health to care team.
• Direct and supervise daily work of Resident Care Assistants.
• Mentor Lead Resident Care Assistants in all aspects of their job.
• Coordinate care needs with community providers via case management.
• Balance service demands with resident independence.
• Recognize and report elder abuse/exploitation per state regulations.
• Respond to emergencies and emergency response systems.
