Collierville Health and Rehab is seeking LPNs and RNs to join our team in Collierville, TN. We offer day and night shifts, a comprehensive benefits package, and opportunities for growth as part of a recognized employer in the healthcare sector.

The role focuses on delivering high-quality patient care, coordinating with the healthcare team, and supporting residents and families. Current TN RN/LPN license is required, with long-term care experience preferred.

Qualifications

• Current RN/LPN license in the state.
• At least one year of LPN or RN experience in a long-term care setting preferred.

Responsabilités

• Consults and coordinates with healthcare team members to assess, plan, implement, and evaluate patient care plans.
• Prepares and administers medications and records prescribed treatments; report adverse reactions as required.
• Records patients' medical information and vital signs.
• Monitors and adjusts equipment and interprets electronic displays.
• Initiates corrective action when patients display adverse symptoms.
• Provides bedside care for a wide variety of medical patients.
• Changes dressings, inserts catheters, starts IVs.
• Prepares equipment and aids the physician during examination and treatment.
• Responds to life-saving situations per nursing standards and protocol.
• Participates in discharge planning.
• Utilizes time management skills.
• Records care information concisely and accurately.
