Job summary

Qualifications

• LPNs with home health intake experience preferred.
• Knowledge of medical terminology and intake processes.

Responsibilities

• Acts as liaison with referral sources and health professionals.
• Receives and coordinates all incoming calls for access to services.
• Completes intake screening and preliminary admission decisions.
• Logs referrals, prepares reports, and enters data for customers seeking care.
• Promotes care/services to prospective patients/clients.
• Supports scheduling and coordination with field staff.
• Handles PHI in compliance with applicable laws.
