Job summary

Interim HealthCare, Lexington, MA, is seeking a Home Health LPN Intake Coordinator to serve patients and referral sources by coordinating intake and initiating care services.

You will handle incoming calls, complete intake screening, assist with scheduling, and promote care services while maintaining compliance with health regulations. A valid LPN license and 3+ years in medical services are required.

Qualifications

• LPN with home health intake experience.
• Three (3) years relevant experience in medical services or clinical environment including intake.

Responsibilities

• Acts as liaison interfacing with referral sources, health professionals and field staff.
• Receives and coordinates all incoming calls, providing information to access services.
• Communicates with patients, clients and families regarding initiation of care/services.
• Completes intake screening and documents information for admission decisions.
• Logs referrals and prepares reports for inquiries and data systems.
• Promotes care/services to prospective patients/clients.
• Helps schedule appointments and coordinates with field staff.
• Obtain verbal orders from physicians where permitted by law to initiate care/services.
• Creates patient files and maintains PHI in accordance with regulations.
• Works with clinical staff to ensure standards of nursing practices and laws.
