Benchmark Senior Living at Orchard Valley in Wilbraham, MA is seeking a Full Time LPN/RN to deliver compassionate nursing care to residents in a warm, home-like setting. Shifts are 2pm-9pm and pay $31-$34 per hour.

You will assess residents, document changes, supervise aides, and collaborate with the care team to support independence and safety, while building meaningful relationships with residents and families.

Qualifications

• Current state license as an LPN and CPR certification.
• Graduate of an approved LPN program (per state requirements).
• Minimum of 1 to 2 years of nursing experience; elderly care preferred.
• Experience working with the elderly in paid or volunteer roles is highly desirable.

Responsibilities

• Accurately assesses, communicates, and documents residents’ status.
• Observes and reports significant changes in resident health to the care team.
• Directs and supervises daily work of Resident Care Assistants.
• Mentors and directs Lead Resident Care Assistants in their duties.
• Coordinates care with community providers via case management.
• Balances service demands with resident independence support.
• Recognizes and reports elder abuse or neglect per state regulations.
• Responds to emergencies and uses the personal emergency system as needed.

Benchmark at Orchard Valley is looking for a Full Time Licensed Practical Nurse (LPN)/Registered Nurse (RN) to join our team!

Full Time 2p-9p

$31-$34/HR

Responsibilities

• Accurately assesses, communicates, and documents residents’ status.
• Observes and reports any significant changes in resident behavior and health to the Resident Care Director, physician, family, RCAs, and Executive Director per state regulations.
• Directs and supervises the daily work assignments of the Resident Care Assistants.
• Closely mentors, guides, and directs the Lead Resident Care Assistants in all aspects of their job.
• Coordinates care needs with community providers via an effective case management process.
• Effectively balances service demands with supporting resident independence.
• Recognizes and assists in the prevention of elder abuse, neglect, and exploitation (financial and other) and reports to appropriate sources per state regulations.
• Responds to emergencies and personal emergency response system and knows when to call for backup.

Requirements

• Current state license as an LPN and CPR certification
• Graduate of an approved LPN program (per state requirements)
• Minimum of 1 to 2 years of experience working as a nurse preferred
• Previous experience working with the elderly in a paid or volunteer position is highly desirable
