Qualifications

• Graduation from an accredited nursing school and a current state license for LPN or RN is required at all times.
• 1+ years of nursing experience preferred but new graduates are welcome.
• CPR or BLS certification preferred.
• Demonstrated proficiency in common digital tools and applications, including Microsoft Office programs (Outlook, Excel, Word), web-based platforms, electronic health record systems, and any HR-related systems as applicable.

Responsibilities

• Provide nursing services aligned with nurse practice act and regulatory standards.
• Oversee care staff direction and oversight with the Director of Nursing.
• Review and update assessments and care plans at admission and as conditions change.
• Administer and document medications following the five rights and MAR documentation.
• Educate residents and families per individualized plans.
• Monitor residents and communicate changes to physicians and proxies.
• Ensure effective communication with families and healthcare teams.
• Support fall prevention and safety initiatives.
• Assist with activities of daily living and scheduling medical appointments.
• Maintain a restraint-free environment and participate in emergency responses as needed.
