Job summary

A healthcare organization in Massachusetts is seeking to fill a position that coordinates anticoagulation management for patients. This role involves working with patients to ensure safe and effective dosage of anticoagulants, as well as documenting treatment plans and patient care issues. The ideal candidate will be a licensed RN or LPN with strong communication skills and experience in anticoagulation. This position requires 32 hours of work per week from Monday to Friday with no weekend or holiday shifts.

Qualifications

• Graduate of an accredited school of nursing.
• Currently licensed as an RN or LPN in Massachusetts.
• Basic Life Support Certification required.
• Strong interpersonal and customer service skills needed.
• Must handle office technology and computer applications efficiently.
• Prior experience in anticoagulation preferred.
• Ability to multitask in a busy clinic with strong communication and phone skills.

Responsibilities

• Coordinates anticoagulation management for patients.
• Monitors lab testing and dosage instructions.

• Provides education regarding anticoagulation.
• Adjust anticoagulation doses per protocol or provider instruction.
• Provide teaching and review safety issues with patients and caregivers.
• Document anticoagulation details in CoagClinic (diagnosis, range, labs, dosages, precautions, follow-up).
• Initiate home INR monitoring as directed by provider.
• Process prescription refills per policy or provider instruction.
• Notify provider of adverse reactions or excessive anticoagulation.
• Coordinate anticoagulation management around procedures with other providers.

Job description

Purpose of Position: Coordinates the Coumadin Clinic in an efficient manner. Collaborates with the provider and patient for coordination of anticoagulation management including; education, instruction and anticoagulation issues.

Description

• Coordinates Coumadin and Lovenox anticoagulation for new and established clinic patients
• Gathers information from provider to obtain anticoagulation needs, diagnosis, desirable range and length of treatment.
• Monitors and coordinates lab testing and dosage instructions with the patient
• Obtains and enters lab orders in accordance with protocol
• Adjusts anticoagulation dose in accordance with the protocol or as instructed by provider
• Provides teaching and reviews safety issues with patients and care givers
• Documents in CoagClinic reason for anticoagulation, diagnosis, desired range, laboratory data, dosage instructions, precautions, and follow up laboratory testing
• Initiates home INR monitor testing with Alere, as instructed by provider
• Processes prescription refills in accordance with refill policy or as instructed by provider
• Notifies provider of any adverse reactions or when excessive anticoagulation occurs
• Coordinates with other providers and patients when anticoagulation needs to be temporarily suspended for procedure
• Works with VNA, other Home Health Groups and independent labs
• Sets up new home draws
• Coordinates bridge therapy with Lovenox as directed
• Tracks delinquent anticoagulation patients and coordinates their follow-up requirements
• Performs other work related duties as assigned or requested.
  
  

Qualifications

• Graduate of an accredited school of nursing
• Currently licensed as an RN (P021) or LPN (P016) in the Commonwealth of Massachusetts
• Basic Life Support Certification for Health Care Providers required
• Strong interpersonal, customer service, time management skills required
• Experience with current office technology, proficient with computer office applications
• Prior work experience and excellent knowledge in anticoagulation preferred
• Ability to multitask in a busy clinic setting including excellent communication and phone skills
