Job summary

Ascension is seeking an LPN for day shift at Ascension St. Vincent's Southside Roger Building in Jacksonville, FL. You will provide direct patient care, administer medications under supervision, and monitor patient safety in collaboration with RNs and physicians.

Responsibilities include patient assessment, performing essential labs and therapies, and educating patients and families about treatment plans, self-care, and post-discharge follow-up to support safe, high-quality care.

Qualifications

• BLS Provider certification required within 1 month of hire.
• LPN licensure in FL or GA required prior to hire.

• Assist with patient assessment and ensure patient safety and well-being.
• Perform essential laboratory tests and therapeutic services.
• Educate patients and families on treatment plans, self-care, holistic health needs, and post-discharge follow-up.

What minimum requirements you’ll need

Licensure / Certification / Registration:

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• One or more of the following required:
• Licensed Practical/Vocational Nurse credentialed from the Georgia Board of Nursing obtained prior to hire date or job transfer date. Valid state license required specific to the state in which the associate works, or Compact state license specific to the state in which the associate works.
• Licensed Practical/Vocational Nurse credentialed from the Florida Board of Nursing obtained prior to hire date or job transfer date. Valid state license required specific to the state in which the associate works, or Compact state license specific to the state in which the associate works.

Education:

• High School diploma equivalency OR 1 year of applicable cumulative job specific experience required.
• Note: Required professional licensure/certification can be used in lieu of education or experience, if applicable.
• Office experience as an LPN
