Job summary

Midwest Health, Inc. seeks an LPN to work with Executive Directors and RCCs to support clinical coordination across Topeka, Kansas City, and Manhattan. Responsibilities include quarterly pharmacy reviews, risk watch, admissions and re-admissions, labs, order entry, and PCP notifications.

The position requires an active LPN license and reliable transportation. You will cover for the RCC during vacations, illness, or vacancies, contributing to a smooth, patient-centered workflow in a collaborative

Role Description

This person will work with the Executive Directors and RCCs (Resident Care Coordinator) to assist with task completion of items such as: quarterly pharmacy reviews, Risk Watch, admissions/re-admissions, labs (routine draw, review completion, and documented results, PCP notification and new orders, in-house physician visits/rounds, and order entry, and covering for the RCC when the RCC is on vacation, sick or if the position is vacant for a period of time.
