Job summary

Road To Responsibility, Inc. is seeking a licensed practical nurse (LPN) to provide general nursing care in a residential services setting.

You will administer medications, monitor vital signs, and support daily living activities for individuals with disabilities. Requirements include an active LPN license, at least one year of related experience, a valid driver’s license, and willingness to undergo background checks.

Qualifications

• Current LPN license.
• Minimum of one year related experience.
• Valid Driver’s License.
• Basic knowledge of Microsoft Word and the internet.
• Successful completion of all state and agency mandated trainings including medication administration rules and regulations.
• Willing to authorize a Criminal History Background Check (CORI) and release of information concerning DPPC, DDS, and other agencies.
• Able to perform each essential duty satisfactorily with reasonable accommodations.

Responsibilities

• Explains procedures and treatments to individuals to gain cooperation and understanding.
• Administers prescribed medication and treatments per MAP Guidelines.
• Maintains awareness of comfort and safety needs of individuals.
• Observes individuals, records significant conditions, and notifies supervisor or physician.
• Takes vital signs and assesses condition.
• Responds to life-saving situations per nursing standards and policies.
• Oversee medication administration within program according to DDS regulations.
• Liaises between DDS, Day Program, and families around medical care and support.
• Completes/updates medical assessments annually or as requested by Manager.
• Keeps monthly progress notes for each participant.
• Assists with personal care tasks as needed (diet, bathing, dressing, etc.).
• Assists with physical direct care (transfers).
• Conducts medical trainings for staff on-site.
• Participates in on-the-job training and coaching of new staff.
• Adheres to RTR policies and procedures.
