Ohio Living Ganzhorn Suites seeks a Licensed Practical Nurse (LPN) to join our care team. The Clinical Director leads resident health and wellness visits, assessments, and coordination of medications.

The role requires an active Ohio license as an RN or LPN and at least one year of nursing experience in a hospital or long-term care setting, with proficiency in Windows, MS Office, and EMR systems preferred.

Formación

• Current unencumbered Ohio license to practice as RN or LPN.
• At least 1 year of nursing experience in hospital or long-term care.
• Proficiency with Windows, MS Office; EMR experience preferred.

Responsabilidades

• Meet residents regularly to provide health and wellness checks.
• Review assessments and move-in medical information for fit and needs.
• Order medications and oversee medication schedules.
• Document medication assistance and maintain wellness files.
• Develop and update individual service plans and communicate with families.
• Be on call for medical questions and emergencies; coordinate dietary needs.
• Coordinate transfers and liaise with physician teams.
