Encore HCS currently seeks compassionate Licensed Practical Nurses (LPN) to join Encore Parkside in Delaware. The role focuses on resident-centered care with supportive leadership and teamwork.

Qualifications

• Current LPN license in Delaware.
• Graduation from an accredited LPN program.

Responsibilities

• Performs resident assessments using direct physical exams, chart reviews, labs, and input from the interdisciplinary team.
• Monitors residents for changes in condition and notifies physicians and families; transcribes orders.
• Administers medications according to facility policy and monitors for therapeutic and adverse effects.
