Job summary

HarborChase of Palm Coast is seeking an LPN/LVN to provide direct nursing care for residents under the supervision of the Director of Resident Care. You will ensure safe care delivery, document thoroughly, and support residents and families as needed. Requirements include graduation from an accredited LPN/LVN program and current Florida licensure, with 1+ year in healthcare, preferably long-term care.

Willingness to work 8 a.m. to 6 p.m.

Qualifications

• Graduated from an accredited LPN/LVN program
• Must be currently registered in Florida
• Experience in long-term care or geriatric settings preferred

Responsibilities

• Provide direct nursing care per physician orders under supervision of DRC
• Document care accurately on MAR and in charts
• Assess residents and communicate information to care team
• Administer medications per physician orders and monitor for adverse reactions
• Maintain safety and confidentiality in a resident-centered environment
• Organize resources to meet residents' needs and assist with self-care activities
