Job summary

Harmony Senior Services in Delaware is seeking an LPN to provide quality nursing care to residents in an assisted living setting. You will coordinate physical, social, and emotional care needs under physician orders and resident choices.

The role requires strong communication, teamwork, and time management; you will participate in medication management, documentation, and staff supervision as part of a safety-focused care team.

Qualifications

• Must hold a current LPN license in the state of Delaware.
• Experience with seniors in assisted living or LTC preferred.
• Strong communication and teamwork abilities.
• Physical capability to lift up to 50 lbs.

Responsibilities

• Provide quality nursing care to residents in a respectful and dignified manner.
• Coordinate physical, social, and emotional aspects of care per physician orders and resident choices.
• Execute the medication management plan with documentation and physician communication as needed.
• Assess resident needs and update care plans as conditions change.
• Respond to emergencies calmly and promptly; document incidents and changes.
• Assist with meals and monitor resident nutrition and acceptance of diets.
• Maintain accurate records and documentation; support CNA/RMA staff.
• Attend trainings, CEUs, and staff meetings; adhere to policies.
