Qualifications

Responsibilities

• Oversee medication administration, guiding and supervising Medication Aides and ensuring medications are administered and documented per regulations and policies.
• Provide clinical oversight and supervision to Caregivers to ensure safe, high-quality care.
• Complete resident assessments and nursing documentation, evaluate health needs and changes in status, maintain clinical records.
• Receive, transcribe, and implement physician orders, carry out provider orders safely and promptly; administer medications or injections as needed.
• Respond to resident health needs and emergencies, assess and intervene, coordinate additional medical services when necessary.
• Coordinate resident care and communication with residents, families, providers, and staff, supporting continuity of care and resident dignity.

Holy Family Senior Living is seeking compassionate and dependable LPN Resident Care Coordinator to join our team in Bethlehem, PA. At Holy Family, we care like family.

What You'll Do as an LPN Resident Care Coordinator:

• Oversee medication administration, providing guidance and supervision to Medication Aides and ensuring medications are administered and documented according to regulations and facility policies.
• Provide clinical oversight and supervision to Caregivers, helping ensure residents receive safe, consistent, high-quality care.
• Complete resident assessments and nursing documentation, evaluating residents' health needs and changes in functional status and maintaining required clinical records.
• Receive, transcribe, and implement physician orders, ensuring provider orders are carried out safely and promptly and administering medications or routine injections as needed.
• Respond to resident health needs and clinical emergencies, assessing residents, providing appropriate nursing interventions, addressing safety concerns, and coordinating additional medical services when necessary.
• Coordinate resident care and communication with residents, families, providers, and staff while supporting continuity of care, resident preferences, dignity, independence, and choice.

LPN Resident Care Coordinator Qualifications:

• Current Licensed Practical Nurse (LPN) license or eligibility for licensure in Pennsylvania
• Current CPR certification or ability to obtain
• High School Diploma or GED
• 1+ year of clinical healthcare experience preferred, ideally in senior living, personal care, nursing, acute care, home care, or hospice
• Strong verbal and written communication skills with the ability to effectively communicate with residents, families, providers, and team members
• Ability to meet the physical requirements of the position
• Comfortable working in a pet-friendly environment
