Job summary

Holy Family Senior Living in Bethlehem, PA is seeking a compassionate LPN to oversee resident care on your assigned unit, administer medications and treatments, and support CNAs with safe, person-centered care.

Qualifications

• Current LPN license in Pennsylvania or valid multistate compact license.
• Graduate of an approved nursing education program.
• Long-term care experience preferred.

Responsibilities

• Provide clinical oversight for the assigned unit and coordinate staff assignments based on residents' care needs.
• Administer and document prescribed medications, treatments, wound care, and other nursing services.
• Assess residents, detect condition changes, initiate interventions, and notify physicians or responsible parties.
• Supervise and coach CNAs to promote quality care, resident safety, and adherence to care plans.
• Complete timely clinical documentation including nursing notes, assessments, incident reports, and shift reports.
• Collaborate with physicians, pharmacies, families, and the interdisciplinary team to coordinate care.

Holy Family Senior Living is seeking compassionate and dependable LPN: Licensed Practical Nurse to join our team in Bethlehem, PA. At Holy Family, we care like family.

As an LPN: Licensed Practical Nurse, you will oversee resident care on your assigned unit, administer medications and treatments, and provide leadership and support to our Certified Nursing Assistants. You will work closely with residents, families, physicians, and fellow team members to deliver safe, responsive, and person-centered care.

What You'll Do as an LPN: Licensed Practical Nurse:

• Provide clinical oversight for your assigned unit and coordinate staff assignments based on residents' care needs
• Administer and accurately document prescribed medications, treatments, wound care, and other nursing services
• Assess residents, recognize changes in condition, initiate appropriate interventions, and notify physicians and responsible parties as needed
• Supervise, coach, and support CNAs while promoting quality care, resident safety, excellent service, and compliance with care plans
• Complete timely and accurate clinical documentation, including nursing notes, assessments, incident reports, and change-of-shift reports
• Collaborate with physicians, pharmacies, families, and interdisciplinary team members to coordinate care and support positive resident outcomes

LPN: Licensed Practical Nurse Qualifications:

• Current Licensed Practical Nurse license issued by the Pennsylvania Board of Nursing, or a valid multistate compact license
• Graduate of an approved nursing education program
• Long-term care experience preferred
