TRITAL Care is seeking compassionate LPNs for PRN positions to provide high-quality nursing care in skilled nursing and long-term care facilities in Pennsylvania. You will administer medications, monitor residents, and collaborate with RNs, physicians, and other professionals to ensure safety and quality of care.

This PRN role requires strong clinical skills, sound judgment, and effective communication in a fast-paced care environment, with shifts based on facility staffing and availability.

Qualifikationen

• Active LPN license in PA or eligible to practice in Pennsylvania.
• Ability to provide safe nursing care within scope of practice and state regulations.
• Knowledge of medication administration, infection control, and resident safety.
• Ability to recognize changes in resident condition and communicate concerns promptly.
• Strong clinical judgment, communication, documentation, and time-management skills.
• Ability to follow physician orders and care plans.

Aufgaben

• Provide nursing care per physician orders, care plans, and facility policies.
• Administer medications and treatments per protocols.
• Monitor residents, obtain vital signs, and report changes to supervising staff.
• Document care accurately in resident records and EHRs.
• Follow infection prevention, fall prevention, and safety procedures.
• Communicate effectively with residents, families, and care team.
