Job summary

TRITAL Care is seeking compassionate LPNs for PRN roles in skilled nursing and long-term care facilities in Georgia. You will provide nursing care per physician orders, monitor residents, administer medications, and collaborate with RNs, physicians, and other healthcare professionals.

The role requires strong clinical judgment, effective communication, and the ability to adapt to a fast-paced care environment while maintaining resident safety and quality of care.

Qualifications

• Must hold a current LPN license valid in Georgia.
• Ability to provide safe nursing care within LPN scope and state regulations.
• Knowledge of medication administration, infection prevention, and resident safety.
• Ability to communicate concerns to healthcare professionals promptly.
• Strong clinical judgment, communication, documentation, and time-management skills.
• Ability to follow physician orders and facility protocols.

Responsibilities

• Provide nursing care per physician orders and resident care plans.
• Administer medications and treatments per protocol.
• Monitor residents' conditions and vital signs.
• Report significant changes to supervising nurse promptly.
• Perform nursing assessments and observations within scope.
• Assist with wound care and treatments as authorized.
• Document care accurately in health records.
• Support infection and fall prevention and other safety initiatives.
• Communicate effectively with residents, families, and care team.
• Participate in care planning and shift handoffs.
