Job summary

Vitality Living is seeking an LPN to join our Stony Brook Community in Louisville, KY. You will administer or assist with medications, collect specimens for lab studies, and communicate with physicians to coordinate care.

You will help maintain resident charts, assist in developing and updating service plans, and support the Wellness Director in meeting residents’ needs while adhering to license requirements and CPR standards.

Qualifications

• Administer or assist with medications and treatments.
• Collect, store, and transport specimens for laboratory studies.
• Communicate with physicians via telephone, fax, and Physician Communication forms.
• Ensure physician orders, laboratory reports, and other data are scanned and filed in resident charts.

• Assist in development and revision of service plans and inform Wellness Director of resident needs.

LPN Requirements

• Administrate or assist residents to self-administer medications and treatments
• Collect, store, and ensure transport of certain specimens for laboratory studies
• Communicate with physicians via telephone, fax, and Physician Communication forms
• Ensure physician orders, laboratory reports, and other data is scanned and filed in resident charts
• Assist in the development and revision of service plans. Inform Wellness Director of current resident needs and preferences as well as successful service approaches
• Perform other duties as directed by Wellness Director

Join Us Today If You Meet The Following Requirements

• Must be at least 21 years old
• Demonstrate ability to communicate effectively in English, both verbally and in writing
• Meet state or provincial health related requirements
• Maintain current professional license and CPR certification if currently LPN/LVN
• Maintain any other certifications as required by state or provincial regulations
