Job summary

Maxim Healthcare in Orange County is hiring a Licensed Practical Nurse (LPN) to work with an adult patient in their own home. The role is full-time, part-time, or PRN with day and night shifts, offering competitive pay and benefits.

You will use the nursing process to plan, implement, and evaluate patient care, initiate plans of care, administer medications, and monitor outcomes in a home-care setting. LPNs must be licensed and current with BLS and TB/Chest X-ray, with prior nursing experience

Qualifications

• Licensed LPN/LVN in the state where practice is required.
• Current TB or Chest X-Ray.
• Current BLS certificate.
• One year of prior professional nursing experience preferred.

Responsibilities

• Uses the nursing process to plan, implement, and evaluate patient care.
• Initiates care plans and updates as patient needs change.
• Performs therapeutic, preventive, and rehabilitative nursing actions.
• Intervenes for changes in patient condition and communicates with physicians.
• Administers medications and documents outcomes.
• Reviews care plans and supports education, coordination, and discharge planning.

Responsibilities

• Utilizes the nursing process to plan, implement, and evaluate patient/client care and/or services provided to client(s) receiving contracted services, as required


• Initiates the plan of care when directed and requests from the prescriber any necessary revisions as the patient's status and needs change


• Incorporates therapeutic, preventive, and rehabilitative nursing actions based on patient diagnosis and established goals


• Intervenes as applicable in response to changes in patient/client condition and reports such changes to the appropriate physician or supervisor


• Administers medications and treatments as prescribed, as well as monitors and documents the effectiveness of the intervention


• Reviews plan of care and program protocols and identifies areas for healthcare teaching/education, care coordination, goal setting, and discharge planning needs, as applicable

Requirements

• Currently licensed as an LPN/LVN in the state in which the LPN will practice.


• Current TB or Chest X-Ray.


• Current BLS card.


• One year of prior professional nursing experience preferred
