Job summary

Acts Retirement-Life Communities in Middletown Township, PA is seeking a qualified Primary Care Office Nurse (LPN) to support residents with person-centered nursing care and coordinate with the Nurse Practitioner and wellness team. The ideal candidate holds an active LPN license, has at least two years of clinical nursing experience (preferably in senior living), and is CPR/AED/First Aid certified or willing to obtain within three months.

Qualifications

• Active LPN license in good standing with state Board of Nursing.
• Minimum of two years clinical nursing experience, preferably in senior living or wellness.
• Prior experience in office management, care coordination, or leadership is a plus.

Responsibilities

• Deliver high-quality nursing care in the Primary Care Office under guidance of the Nurse Practitioner.
• Support proactive health monitoring and access to wellness resources.
• Collaborate with wellness team to promote resident independence, dignity and thriving in place.
