Job summary

Acts Retirement-Life Communities is seeking a Primary Care Office Nurse (LPN) to support residents' health and well-being through high-quality nursing services within the Primary Care Office. You will collaborate with the Nurse Practitioner and wellness team to deliver person-directed care and promote independence in place.

Qualifications

• Active Licensed Practical Nurse (LPN) license in good standing with the state Board of Nursing.
• Minimum of two years of clinical nursing experience, preferably in senior living or wellness-focused setting.
• Prior experience in office management, care coordination, or leadership roles preferred.
• Current certification in CPR, AED, and first aid (or obtain within three months of employment).

Responsibilities

• Provide nursing services under the supervision of the Nurse Practitioner and wellness team.
• Support proactive health monitoring and care coordination for residents.
• Assist in interdisciplinary care planning and facilitate access to well-being resources.
• Foster a resident-centered approach to promote independence and thriving in place.
