Job summary

Community Health & Dental Care, Inc. seeks a Licensed Practical Nurse to provide direct patient care in an outpatient setting. The LPN will assist providers, manage screenings and education, and document care in the EHR across CHDC sites.

The role requires Pennsylvania LPN licensure, graduation from an accredited Practical Nursing program, and CPR certification, with travel as needed to supported CHDC sites.

Qualifications

• Minimum one year of experience in medical practice, ambulatory or primary care preferred.
• Knowledge of nursing principles, medical office operations, and clinical procedures.
• Experience with Electronic Health Records (EHR) required, EPIC preferred.

Responsibilities

• Prepare patients for provider visits by documenting vital signs and medical history updates.
• Document patient information and clinical findings in the EHR.
• Assist physicians with examinations, procedures, and treatments.
