Healthcare Deciphered seeks a licensed Practical Nurse (LPN) to provide direct patient care under RN supervision across PA, MD, DE, DC, VA, and more. The role emphasizes care planning, med administration, and patient education.

Qualifications

• Graduate of an approved Practical/Vocational Nursing program.
• Valid LPN license in the state(s) of practice.
• Current BLS/CPR certification.
• Experience in rehabilitative or geriatric nursing preferred.
• Must provide verification of TB skin test (TST) as required by law.
• Must be able to lift up to 50 lbs and stand/walk long periods.
• Compliance with HIPAA and healthcare regulations.

Responsibilities

• Patient evaluation: collect, report, and document patient conditions.
• Care planning: develop and implement individualized patient care plans.
• Direct patient care: administer medications and treatments per physician orders.
• Care management: coordinate discharge planning and educate patients and families.
• Supervision of staff: monitor and assign duties to unlicensed healthcare staff as per regulations.
