Job summary

Creekside Center for Rehabilitation and Healing is seeking an LPN to join our post-acute care team. You will administer medications, assist with resident admissions and discharges, and document care in the medical records while upholding confidentiality and DOH standards.

Available shifts include day and night, with full-time, part-time, or per diem positions. We offer competitive pay up to $33/hour, insurance, and employee perks to support your professional growth.

Qualifications

• Must possess an LPN degree from an accredited program or be a graduate of an approved LPN/RN program.
• Strong ability to administer medications and monitor residents’ responses.
• Effective communication with residents, families and the care team.

Responsibilities

• Medication administration and treatments.
• Assist with admission, transfer and discharge of residents.
• Document resident care services by charting in resident medical records and department records.
• Educate residents and families regarding treatment and changes of status.
• Adhere to infection control policies and facility policies.
• Consult with resident’s physician regarding plan of care and changes.
• Maintain resident confidentiality and privacy.
• Support interdisciplinary team to solve resident problems.
