Creekside Center for Rehabilitation and Healing is seeking an LPN to provide high-quality patient care in a post-acute setting. The role includes medication administration, documentation, and education for residents and families.

Available shifts include Day and Night, with full-time, part-time, and per diem options. The facility emphasizes compliance with DOH standards and interdisciplinary collaboration to ensure optimal resident outcomes.

Qualifications

• Must have a nursing degree from an accredited college or university.
• Graduate of an approved LPN or RN program.
• Possession of an active LPN or RN license or eligibility to obtain one.

Responsibilities

• Administer medications and treatments.
• Assist with admissions, transfers and discharges.
• Document resident care in medical records and department records.
• Educate residents and families about treatments and status changes.
• Adhere to infection control policies and facility procedures.
• Consult with residents' physicians about care plans and changes.
• Maintain confidentiality and privacy of residents.
• Ensure quality of care by following DOH standards and facility policies.
• Collaborate with interdisciplinary team to solve residents' problems.
