Brookhaven Health Care Facility in East Patchogue, NY is seeking a Licensed Practical Nurse (LPN) to join its pool nursing team. You will administer medications, provide high-quality nursing care, and support resident safety and comfort in a team-focused environment.

Qualifications

• Must have NYS Licensed Practical Nurse (LPN) license.
• Valid CPR certification is required.
• High School Diploma or equivalent is required; Associate's degree preferred.
• Experience in a skilled nursing facility or geriatric care is preferred.
• Strong clinical, communication, and organizational skills.

Responsibilities

• Provide nursing care following resident assessments and care plans.
• Administer medications, treatments, enteral feedings, and IV therapies within LPN scope.
• Monitor resident conditions and report changes promptly to providers and supervisors.
• Document care, treatments, observations, and medication administration accurately.
• Supervise nursing assistants to ensure quality resident care.
• Maintain up-to-date resident care information and communicate during shift changes.
• Follow infection control, safety, and resident rights protocols.
• Participate in interdisciplinary care planning and quality improvement initiatives.
• Assist with admissions, transfers, and other nursing duties as assigned.

Licensed Practical Nurse (LPN)- Pool
Rate: $40/hour
Shift: Full-Time, Part-Time
Schedule: Hiring all Shifts

RESPONSIBILITIES:

• Provide nursing care in accordance with resident assessments, care plans, and facility policies
• Administer medications, treatments, enteral feedings, and IV therapies within the LPN scope of practice
• Monitor resident conditions and promptly communicate changes to providers, supervisors, and responsible parties
• Accurately document care, treatments, observations, and medication administration
• Supervise and support Nursing Assistants to ensure quality resident care
• Maintain up-to-date resident care information and communicate pertinent information during shift changes
• Follow infection control, safety, resident rights, and confidentiality standards
• Participate in interdisciplinary care planning and quality improvement initiatives
• Assist with admissions, transfers, and other nursing responsibilities as assigned

REQUIREMENTS:

• High School Diploma or equivalent required; Associate's Degree preferred
• Valid NYS Licensed Practical Nurse (LPN) License required
• Current CPR Certification required
• Experience in a skilled nursing facility or with the geriatric population preferred
• Strong clinical, communication, and organizational skills
• Knowledge of medication administration, infection control, and resident care practices
• Ability to supervise nursing assistants and work effectively as part of an interdisciplinary team
• Compassionate, resident-centered approach to care
