Qualifications

• Valid NYS LPN license required.
• High School Diploma or equivalent; Associate’s Degree preferred.
• Current CPR certification required.
• Experience in skilled nursing facility or geriatric population preferred.
• Strong clinical, communication, and organizational skills.
• Ability to supervise nursing assistants and work in an interdisciplinary team.

Responsibilities

• Provide nursing care in accordance with assessments, care plans, and policies.
• Administer medications, treatments, enteral feedings, and IV therapies within LPN scope.
• Monitor resident conditions and communicate changes to providers and supervisors.
• Accurately document care, treatments, observations, and medications.
• Supervise nursing assistants to ensure quality resident care.
• Maintain up-to-date information and communicate during shift changes.
• Follow infection control, safety, and confidentiality standards.
• Participate in interdisciplinary care planning and quality initiatives.
• Assist with admissions, transfers, and other nursing responsibilities.

Schedule: Hiring all shifts

Rate: $40/hour

Shift: Full‑Time, Part‑Time

• Administer medications, treatments, enteral feedings, and IV therapies within the LPN scope of practice
• Monitor resident conditions and promptly communicate changes to providers, supervisors, and responsible parties
• Accurately document care, treatments, observations, and medication administration
• Supervise and support Nursing Assistants to ensure quality resident care
• Maintain up‑to‑date resident care information and communicate pertinent information during shift changes
• Follow infection control, safety, resident rights, and confidentiality standards
• Participate in interdisciplinary care planning and quality improvement initiatives
• Assist with admissions, transfers, and other nursing responsibilities as assigned

Requirements

• High School Diploma or equivalent required; Associate’s Degree preferred
• Valid NYS Licensed Practical Nurse (LPN) License required
• Current CPR Certification required
• Experience in a skilled nursing facility or with the geriatric population preferred
• Strong clinical, communication, and organizational skills
• Knowledge of medication administration, infection control, and resident care practices
• Ability to supervise nursing assistants and work effectively as part of an interdisciplinary team
• Compassionate, resident‑centered approach to care
