Job summary

Matrix Home Health Care seeks an experienced Licensed Practical Nurse (LPN) for per diem home health visits in the Boynton/Lake Worth, FL area. The role initiates patient care, performs comprehensive assessments, and ensures accurate OASIS documentation for home health patients.

You will work with a team of clinicians to deliver skilled nursing services, educate patients and families, and coordinate care across physicians and care managers.

Qualifications

• Active Florida LPN license in good standing.
• Minimum 1 year of home health experience.
• Strong experience with OASIS documentation.
• Experience with KanTime EMR system preferred.
• Excellent clinical assessment and critical thinking skills.
• Ability to work independently and manage a flexible schedule.
• Reliable transportation and valid driver’s license.

Responsibilities

• Provide home-based care to patients in the Boynton/Lake Worth, FL area.
• Examine patients, complete clinical assessments, document progress, and report changes to physicians.
• Complete accurate clinical documentation including OASIS and care coordination notes.
• Develop, implement, and monitor patient care plans with patients and their representatives.
• Deliver skilled nursing services and provide case management for assigned patients.

Job Description

Matrix Home Care is seeking experienced and compassionate Licensed Practical Nurses (LPN) to perform home health visits in the Boynton/Lake Worth , FL area . This role is critical in initiating patient care, completing comprehensive assessments, and ensuring accurate OASIS documentation for home health patients.

• Provide home‑based care to patients with illnesses, injuries, and chronic conditions, often following discharge from a hospital, rehabilitation center, or nursing facility
• Examine patients, complete clinical assessments, document progress, and report changes to physicians
  
• Complete accurate and timely clinical documentation, including OASIS, evaluations, physician orders, and care coordination notes
  
• Develop, implement, and continuously monitor patient care plans in collaboration with patients and their representatives
