What you’ll be doing:

1. Perform comprehensive assessments of client status, including physical, psychosocial, and environmental parameters.
2. Develop individualized care plans based on data analysis and scientific findings.
3. Provide interventions focused on achieving realistic client outcomes.
4. Report changes to update the care plan reflecting progress.
5. Administer medications as needed.
6. Maintain client records of assessments, interventions, and evaluations.
7. Initiate referrals to other health team members as needed.

What we’re looking for:

1. Graduate of an accredited nursing school.
2. Current license by SC State Board of Nursing as a LPN or RN.
3. Current CPR certification.
4. Reliable transportation.
5. Confidentiality in all job aspects.
6. Knowledge of safety and infection control practices.
