What you’ll be doing:

1. Performing comprehensive assessments of client status, including physical, psychosocial, and environmental parameters.
2. Developing an individualized plan of care for each client based on data analysis and current scientific findings.
3. Providing interventions focused on achieving realistic client outcomes.
4. Reporting changes to update the plan of care reflecting progress towards goals.
5. Administering medications as needed.
6. Maintaining client records of assessments, planning, and evaluations.
7. Initiating referrals to other health team members as needed.

What we’re looking for:

1. Graduate of an accredited nursing school.
2. Current license by SC State Board of Nursing as an LPN or RN.
3. Current CPR certification.
4. Reliable transportation.
5. Ability to maintain confidentiality.
6. Knowledge of safety and infection control practices.
