What you’ll be doing:

1. Perform comprehensive assessments of client status, including physical, psychosocial, and environmental parameters.
2. Develop individualized care plans based on data analysis and current scientific findings.
3. Provide interventions aimed at achieving realistic client outcomes.
4. Report changes and update care plans to reflect progress towards goals.
5. Administer medications as needed.
6. Maintain detailed client records including assessments, interventions, and evaluations.
7. Initiate referrals to other health team members when necessary.

What we’re looking for:

1. Graduate of an accredited nursing school.
2. Current license by SC State Board of Nursing as an LPN or RN.
3. Current CPR certification.
4. Reliable transportation.
5. Ability to maintain confidentiality.
6. Knowledge of safety and infection control practices.
