Job summary

Heartland Community Health Center is seeking a Licensed Practical Nurse (LPN) to provide patient-centered nursing care in a Federally Qualified Community Health Center. This non-exempt, hourly position supports flexible staffing needs and collaborates with RNs and providers to ensure safe, effective care.

Qualifications

• Current Kansas LPN license.
• Current BLS certification.
• Ambulatory care experience preferred; competency in EHR use.

Responsibilities

• Collect focused patient information and document changes.
• Implement established plans and report outcomes to RN/provider.
• Support patient intake and triage with RN/provider as needed.
• Collaborate with care team to coordinate patient care.
