Job Purpose

The Licensed Practical Nurse (LPN) supports the delivery of safe, effective, and compassionate patient centered care by providing nursing services within the LPN’s scope of practice, organizational policies, and applicable state and federal requirements at a Federally Qualified Community Health Center. This position provides flexible clinical staffing support to meet patient care needs, maintain access to services, and support continuity of operations during periods of increased demand, employee absences, or other staffing needs.

Through collaboration with providers, registered nurses, and other members of the care team, the LPN supports continuous improvement in the quality, safety, accessibility, and effectiveness of patient care.

The LPN collaborates with providers, nurses, medical assistants, and other members of the care team to support patient assessment, care coordination, treatment implementation, patient education, and timely and accurate documentation. • Follow established clinical protocols, standing orders, medication safety procedures, and applicable supervision and delegation requirements; promptly elevate concerns beyond the LPN’s scope or authority.
  • Maintain required licensure, certifications, and competencies; participate in required training and demonstrate professionalism, accountability, and respectful communication.
