Qualifications

• Graduation from an approved Practical Nurse program.
• Current NC LPN licensure or Nurse Licensure Compact (NLC). BLS required.
• Telephone Triage experience preferred.

Responsibilities

• Answer and manage incoming patient telephone calls in a professional and timely manner.
• Assess patient concerns, symptoms, and needs within LPN scope of practice and clinical protocols.
• Document all patient communications, assessments, interventions, and follow-ups in the EMR.
• Schedule, reschedule, and coordinate patient appointments.
• Provide patient education and care coordination in collaboration with providers and the healthcare team.
• Perform nurse visits and routine procedures within scope of practice (e.g., wound care, suture removal, staple removal, drain removal).
• Maintain patient confidentiality and adhere to organizational policies and standards.
