Job summary

WeCare Centers is seeking a Licensed Practical Nurse (LPN) to join our Avalon Care Center team in New Castle, PA. This as-needed role supports our residents with quality nursing care and collaborative care planning.

The LPN will monitor patient health, administer medications, document care, and work under the supervision of a Registered Nurse to ensure safe, compassionate and compliant service delivery. We offer competitive pay and growth opportunities.

Qualifications

• Valid LPN license in the State of PA.
• Ability to monitor patient health and administer medications.
• Excellent teamwork and communication with healthcare staff.

Responsibilities

• Conduct thorough assessments of each patient’s medical status upon admissions and throughout the course of treatment.
• Provide regular patient status updates.
• Assist in implementing patient’s treatment plan under direction of the Registered Nurse.
• Complete required documentation in an accurate and timely manner.
• Supervise, administer and or assist with medications.
• Provide direct care of residents as necessary.
• Follow the plan of care for each resident.
• Ensure the residents care standards are maintained.
• Report to the Director of Nursing.
