Job summary

WeCare at Avalon Care Center, located at 3410 West Pittsburg Road, New Castle, PA, 16101, is actively seeking dedicated LPNs to join our growing team. As Needed shifts offer flexibility while delivering essential resident care and medication administration.

We value teamwork, supportive leadership, and opportunities for continuing education in a caring, patient-centered environment. Our LPNs monitor patient health, assist with daily living activities, and collaborate with RNs to implement care

Qualifications

• Valid LPN license in Pennsylvania.

Responsibilities

• Assess patients' medical status on admission and throughout care.
• Provide regular patient status updates.
• Assist with treatment plans under RN direction.
• Complete accurate nursing documentation.
• Supervise and assist with medications.
• Provide direct resident care as needed.
• Follow the plan of care for each resident.
• Report to the Director of Nursing.

WeCare at Avalon Care Center, located at 3410 West Pittsburg Road, New Castle, PA, 16101 is actively seeking dedicated LPNs to add to our growing team!

Our LPN's provide essential nursing care, including monitoring patient health, administering medications, assisting with daily activities, and collaborating with the healthcare team to ensure quality resident care.

Job Duties

• Conduct thorough assessments of each patient’s medical status upon admissions and throughout the course of treatment.
• Provide regular patient status updates.
• Assist in implementing patient’s treatment plan under direction of the Registered Nurse.
• Complete required documentation in an accurate and timely manner.
• Supervise, administer and or assist with medications.
• Provide direct care of residents as necessary.
• Follow the plan of care for each resident.
• Ensure the residents care standards are maintained.
• Report to the Director of Nursing.

Qualifications
