Qualifications

• LPN license required in Pennsylvania.
• Technical school diploma (LPN program).
• American Heart Association Basic Life Support certification required upon hire.
• Ability to lift and carry 25 lbs and perform patient care tasks.

Responsibilities

• Provide care to designated patients and coordinate care across the continuum.
• Assist providers during procedures and examinations and clean instruments and rooms.
• Perform administrative functions in the physician practice.
• Collect patient history and reason for visit.
• Track and log ordered laboratory and diagnostic tests and present results to the provider.
• Educate patients and evaluate the effectiveness of patient education.
• Assist with prescription refills, appointment scheduling, and referral authorization inquiries.
• Coordinate internal and external appointments based on patients' medical needs, preferences, and practice schedule.
• Identify and address actual or potential patient risks.
• Participate in telephone triage and advise patients according to established protocols or provider orders.
