Job summary

Signature HealthCARE of Carrollton Rehab & Wellness Center is seeking a dedicated LPN/LVN to provide skilled nursing care, support residents with ADLs, and supervise nursing staff. You will ensure accurate documentation, coordinate care, and participate in care planning within a team environment.

The role requires an active LPN/LVN license, CPR/BLS certification preferred, and at least 1 year of clinical nursing experience, ideally in long-term care.

Qualifications

• Current, active LPN/LVN license in the state of practice.
• Minimum one (1) year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role; strong documentation skills.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Supervise nursing assistants and medication aides during shifts.
• Monitor residents' condition and communicate changes to physicians and families.
• Develop resident care plans with the interdisciplinary team and document accurately.
