Job summary

Signature HealthCARE in Carrollton is seeking night shift LPNs/LVNs to provide direct nursing care, manage medications, and supervise aides. This role emphasizes patient comfort, safety, and adherence to regulatory standards in a busy long-term care setting.

The ideal candidate holds an active LPN/LVN license in Kentucky, with 1 year of clinical nursing experience. CPR/BLS is preferred, and familiarity with EHR systems is a plus. A generous sign-on bonus and comprehensive benefits are offered.

Qualifications

• Current, active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one (1) year of clinical nursing experience; long-term care experience, preferred.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations, as applicable.
• Monitor and observe residents’ physical and emotional status; notify the attending physician and responsible party of any change in condition.
• Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.

What you Need to make a Difference
