Job summary

Signature HealthCARE Hartford Rehab & Wellness Center is seeking a dedicated LPN/LVN for a full-time, on-site position in a 110-bed facility. You will provide direct nursing care, administer medications, assist with ADLs, and supervise nursing assistants during shifts per policy.

The role requires an active LPN/LVN license, CPR/BLS preferred, and at least one year of clinical nursing experience, preferably in long-term care. Strong observation, documentation, and EHR skills are essential.

Qualifications

• Current, active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Knowledge of Federal and State long-term care regulations.
• Proficiency with electronic health records (EHR) and Microsoft Office.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with activities of daily living (ADLs) while ensuring comfort, dignity, and safety.
• Supervise and direct nursing assistants and medication aides during the shift.
• Monitor residents' status and notify when changes occur.
• Develop, implement, and review individualized resident care plans with the interdisciplinary team.
• Complete accurate clinical documentation per facility policy.
• Coordinate resident admissions, transfers, and discharges according to procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys.
