Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current CPR certification or ability to obtain within 90 days is preferred.
• Ability to manage multiple tasks and work independently with good communication and organization.

Responsibilities

• Provides high quality clinical services within the scope of practice as defined by state laws and plan of care.
• Documents patient visits thoroughly in the electronic medical record.
• Participates in care coordination and communicates with patients, families, physicians and team.
• Ability to work in diverse home settings with respect to age, race, creed, color, sex, disability, or financial condition.
