Job summary

Penn Medicine is seeking a Licensed Practical Nurse to provide unit-based staff support with focus on implementing IBPs and transferring skills across shifts. The LPN will model effective communication, reinforce routines, assist with behavior tracking, and support consistency under supervision of unit leadership.

The role requires LPN licensure, BCLS, and CPR within 30 days of hire. A high school diploma or GED is required. The position is full-time and based in a hospital setting.

Qualifications

• Licensed Practical Nurse (Required)
• Current LPN licensure from PA State Board accredited program
• Basic Cardiac Life Support (Required)
• CPR certification required within 30 days of hire
• Education: HS Diploma or GED (Required)

Responsibilities

• Gathers relevant patient information for nursing assessment and assists in developing the plan of care.
• Models and reinforces IBP strategies during routine care (ADLs, toileting, bathing, meals, transitions).
• Uses agreed-upon scripts and de-escalation steps.
• Supports early identification of triggers and warning signs.
• Collects objective behavior data using approved tools.
• Coaches peers at the bedside under G-BSC guidance and orients staff to resident IBP strategies.
• Participates in huddles/case conferences as assigned.
• Relays frontline observations to inform plan updates.
• Maintains resident dignity, privacy, and safety; follows all policies and standards.
• Assists in implementing patient/family teaching plans.
