Job summary

The Clare in Chicago seeks a compassionate Part-Time LPN to join our Assisted Living/Memory Care Unit. You will provide high-quality nursing care, support ADLs, and monitor residents under physician orders during evening and night shifts.

Responsibilities include updating care plans, reporting to the Director of Health Services, and coordinating with team members to ensure safe, respectful, and individualized care. Night differential applies.

Qualifications

• Current nursing license valid in Illinois.
• CPR certification required.

• Identify resident medical and behavioral changes and support care plan updates.
• Provide daily reports to Health Services on conditions and needs for nursing supplies.

Part-Time LPN

Shift: Week 1 - Friday 6:30 pm to 7:00 am, Week 2 - Monday 6:30 pm to 7:00 am

If so, then The Clare, the only Continuing Care Retirement Community in the Gold Coast area, wants you! We are seeking to fill a Part-Time LPN role in our Assisted Living/Memory Care Unit.

Here are a few of the daily responsibilities of an LPN:

• Assist residents with Activities of Daily Living (ADL), family/social activities and actively promote independent lifestyle of residents.
• Responsible for identification of resident medical and behavioral changes and the development and monitoring of care plans and re-assessment of each resident.
• Provide daily report to the Director of Health Services concerning community conditions, personnel performance, resident conditions, and needs for nursing supplies.
• Provide nursing care according to physician’s orders and in conformance with recognized nursing practice, established standards, and administrative policies.
• Ensure preventative, therapeutic and rehabilitative services to residents and assists in the coordination of nursing care with care rendered by other disciplines.
