Job summary

Rochester Regional Health is seeking an Licensed Practical Nurse (LPN) for ElderONE - Hudson. The position supports care in a landmark PACE program, with direct patient care under R.N. supervision and a home base in Rochester, NY.

40 hours weekly, day or evening shifts, every other weekend required. Responsibilities include patient observation, assisting in treatment plans, nursing services, documentation, and status updates.

Qualifications

• Graduate from an accredited School of Practical Nursing.
• Current LPN licensure in New York.
• BLS Certification from the American Heart Association.

Responsibilities

• Observe and document patient status on admission and throughout treatment.
• Assist in implementing the treatment plan under the RN.
• Provide routine nursing services as directed by the RN.
• Follow department evaluation, treatment, and documentation guidelines.
• Complete required documentation accurately and timely.
• Provide regular patient status updates to appropriate personnel.
• Attend department/facility meetings as required.
• Comply with hospital, regulatory and professional standards.
• Maintain clinical competency through continuing education.
• Perform other duties as assigned.

Education
