Job summary

Oakwood Community Living Center is seeking a licensed practical nurse (LPN) to join our team in a full-time night shift from 6pm to 6am in Dyersburg, TN. The successful candidate will provide care, administer medications, assist with activities of daily living, and document care plans.

Qualifications

• LPN license in good standing required.
• BLS certification required.

Responsibilities

• Evaluation and delivery of care including medication administration, treatments, ADL care, etc., as ordered by physician and per plan of care.
• Conduct regular rounds to monitor resident activity and ensure quality care.
• Communicate with residents, family members, physicians and supervisor regarding resident status.
• Maintain up-to-date medical records during tour of duty on the assigned unit.
• Other tasks as required.
